Please only fill this form out after booking your birthday party and receiving instruction via email to move on to final payment. 

Animal Exploration Party, Fun and Play: Train Ride Party, Pony Ride Party, and Little Gardener Party are based on 20 children total. Any additional kids must be added as "Additional child."

All other party packages are based on 20 guests total (children and adults) so you must add any additional guests beyond the 20 guest total as "additional children" and "additional adults" for your party packages.

The "shipping address" is just a requirement for checkout. Your pizza will be delivered right to your party!

You will be given wristbands for all of your guests. If you have additional guests the day of your party, additional wristbands should be purchased when you arrive.
